Top fastener of the no u-turns sign is missing, causing sign to display upside down

Q. What type of sign is it?

Q. Please describe the location and issue. (i.e. Intersection, Cross Street, etc)
Α. On southbound Creedmoor Road, at intersection with Glenwood Ave/US70 (facing Crabtree Valley Mall). The sign post is in the roadway island next to the left side left turn lane for Creemoor traffic entering Glenwood.
